PERSONAL KNOWLEDGE CANCER SURVEY
Part 1 of 4: About the Survey and General Questions
Access to cancer screening and care can be inconsistent and difficult. In particular, Indigenous families in Prince Albert face a multitude of personal, situational, economic, environmental, and systemic barriers to cancer services and support. Our goal is to engage cancer survivors and their families in a conversation around some of the barriers and challenges they experienced in accessing care. We ask you to consider answering the questions in this survey. Your completion of this survey is voluntary. In the final analysis, your responses will remain confidential and anonymous. Thank you.
The inputs from community members are highly valued to inform the project. We are using two separate surveys to obtain the community input. Please feel free to fill in both surveys as you deem appropriate. A Personal Knowledge Cancer Survey (which is this survey) for every community member to share her or his knowledge. The other survey is a Professional Knowledge Cancer Survey for community members who are working in the human service delivery sector (www.surveymonkey.com/r/QCPXHBC).
