We need your help! Tell us what you think. This survey is sponsored by the Michigan Thumb Public Health Alliance. The Alliance is a collaboration of the Huron, Lapeer, Sanilac, & Tuscola Health Departments.


Complete the survey if you live or work in the Thumb by 9/25/2026.

If you live in Huron, Lapeer, Sanilac, or Tuscola County, you will be entered into a drawing for each section you complete. Up to 5 chances to win a $50 Gift Card. 40 cards will be given away. Limit one card per person.

Eligibility: Only one card per person. Must reside in Michigan in Huron, Lapeer, Sanilac, or Tuscola Counties. Gift cards will only be mailed to addresses in those counties.
