In an effort to inform our Back to School Planning committee, please complete this short survey regarding returning to school in the fall. This information will help us better meet the demands on technology and staffing.

If you have more than one PreK-12 student, please take the survey more than once to represent your number of children.

Responses are confidential.

Question Title

* 1. Please select the grade your child will enter in the fall.

Question Title

* 2. Some students may be high-risk or have family members with medical conditions that make returning to school in the fall concerning. If the CDC and MN Health Department guidelines indicate it is safe for schools to operate with an in-person delivery method of instruction (traditional school day with some modifications to spacing and safety protocols), WILL YOUR CHILD RETURN TO SCHOOL IN THE FALL?

Question Title

* 3. If the school is required to utilize the hybrid learning model, this new model would require schools to alternate learning days for our students. The current model the school is examining would include every other day in-person instruction for K-12, (by household). Preschool would maintain their normal schedule. On days where your child does not report to school they will still have daily school responsibilities for each course delivered electronically and can still participate in school meals, but as a grab and go. State guidance identifies that this model would be implemented if local, regional, or statewide COVID-19 metrics worsen or if a school experiences clusters of cases.

Would your child (K-12) attend in-person instruction, if the district had to implement the hybrid learning model?

Question Title

* 4. Busing is a complex service that may not be able to meet social distancing guidelines. The bus company will work to spread children around the bus (by family) to the greatest extent.

If your child is currently riding a Sleepy Eye bus, will they continue to ride the bus again in the fall, or do you intend to find an alternative ride for your children?

Question Title

* 5. Does your child have access to WiFi/Internet connectivity at home?
