Survey of Health Professionals with Experience in Post-Polio Patient Care

Post-Polio Health International, St. Louis, Missouri, is surveying the health professionals (both practicing and retired) who offer(ed) post-polio care. The results will be shared at the Polio Australia conference in September 2016 and in Post-Polio Health. (This survey is a companion to our Spring 2014 survey of polio survivors about their polio care from post-polio specialists and primary care providers. See pages 9 and 10. The results were compiled and reported.)

The first part of the survey asks general questions about you and your practice. The second section asks questions about the post-polio care you provided. All of the questions are about your experience. The survey should take about 15 minutes.
