1. Information About this Survey

The member organizations of the St. Clair County Health Commission would like your opinion about quality of life in our county. Please take a few minutes to complete this brief survey. It is completely confidential and voluntary, but would help agencies plan health and other human services in the county.

We are looking for a representation of citizens from all of the communities within the St. Clair County area (approximately 2,000 to 3,000 individuals or 1% of the population) to answer these questions.

The results of this survey will be summarized and used to evaluate citizen needs, plan new programs, and monitor progress of county health initiatives. There is no direct, immediate benefit to you for completing this survey. However, as a citizen of the county, you may receive benefits from new or expanded programs in the future.

Your participation is voluntary and will take less than 15 minutes.

Since this is an anonymous survey, please do not put your name on the survey. 
 
Once the survey period is completed, members of the Health Care Commission will meet in July to review the results from this and other community health assessment tools so that we can engage community leaders in a series of discussions to prioritize our most significant health issues and develop strategic health improvement plans for the next five years.  

There are no risks to you for completing this survey, and if you do not want to participate, this will not change anything about the kinds of services you receive from the health department or any other healthcare facility.
