Now that you have participated in the week, please take the time to complete this questionnaire. The purpose of this questionnaire is to gather results or outcomes achieved through the Open Minds Program. There are 6 statements about the change you experienced by participating in Open Minds.
The aggregated data from the questionnaire will assist in determining how the Program is doing and what adjustments can be made in its deliver. We have included some blank lines under each question so that you can provide us with some explanation of why you selected a particular response, a description of how you achieved the particular outcome, as well as some description of how the Program can better meet your needs related to each outcome and be adjusted.
