Vroom Pre-Pilot Survey - Grand Prairie ISD

Thank you for joining other parents participating in the Vroom initiative. One important part of the initiative is finding out what you think about Vroom. To get your thoughts, we are asking you to complete two surveys, the first one before you begin using Vroom, and the second one after the Vroom pilot ends in February 2016. We think each of these surveys will take 15 minutes to complete. You can still participate in the Vroom pilot if you decide not to complete the surveys. However, the information from the surveys will help us improve how Vroom is used by you and other parents in the future.

There are no known risks to you in participating in the Vroom program or in taking these surveys. You may learn some new ways to interact with your child(ren).

CONFIDENTIALITY and ANONYMITY
All information you share with us is confidential. Information gathered through the surveys will be summarized and exclude all references to any individual names. The survey summaries may be shared with others, but we will not share your name or your individual survey answers with anyone (other than those administering and summarizing the surveys who are obligated to keep all personal information confidential). We will protect your privacy. All information collected will only be used for research purposes.

YOUR RIGHTS AS A PARTICIPANT
Participation in this study is voluntary. You have the right not to participate at all or to leave the Vroom pilot at any time. If you do not wish to participate, it will not harm your relationship in any way with the organization from which you learned about Vroom. If you wish to withdraw, however, please let the organization know so we can remove your name from our list of participants taking the survey.
