Independent Living Questionnaire

Hi,

Videre Consulting have been engaged by Phillip Island Homes for the Aged (Melaleuca) to identify what is important to existing and potential residents in Independent Living options. 

This information will be used to contribute to a design brief that will be used to develop the existing and future facilities in a manner that will ensure Melaleuca provides facilities that are valued by its residents, family members, and the community.

The information gathered will be held confidentially, and reported back to the Management Committee in an aggregated format, deidentifying all respondents.

The survey will take approximately 10 minutes to complete.
