Request for Endorsement from SCCM

Please complete the form to submit a Endorsement request to SCCM. 

SCCM requires a minimum of 45 days to conduct the internal review. SCCM will conduct a internal review to ensure that the guideline aligns with SCCM’s mission. If endorsement is approved, the guideline-originating organization may list SCCM as an endorsing organization, not within the title but within the body of the document.

4.SCCM Liaison Appointment as Applicable
5.Please Upload all the Following Documents: 
  • Final Manuscript 
  • Pertinent COI information and how any conflicts were adjudicated
  • A Cover letter that outlines what the organization is looking for from SCCM
  • Other supplement materials
