Canadian Association for Community Living -&nbsp;Survey 2 for&nbsp;Service Providers

The Canadian Association for Community Living (CACL) launched the <a href="http://www.myhomemycommunity.ca" rel="nofollow" target="_blank">My Home My Community</a> initiative to address the need for housing options for individuals with developmental disabilities. Part of this initiative involves identifying and studying existing assets in the community that might be used to create new suitable affordable housing options for this group. This survey, administered by the team at <a href="http://www.shs-inc.ca" rel="nofollow" target="_blank">SHS Consulting</a>, aims to collect some of this asset inventory information. Your answers will be an important contribution to this effort and will help CACL identify new pathways to housing for individuals with a developmental disability. Your participation in this survey is voluntary. All responses will be used for the purposes of this study.
