The equipment at the Learning Commons is intended for educational purposes and is typically loaned on a first come, first serve basis. If you need equipment to complete a class project or class assignment and would like for the Learning Commons to set aside equipment for you please fill out this form completely. Only fully completed forms will be considered. Please understand that this is a request and does not guarantee that equipment will be set aside for you. Requests will be granted based on equipment availability and nature of the assignment. It is recommended that requests are submitted at least one week prior to date the equipment is needed. It is the responsibility of the patron to know the terms and conditions in the Equipment Loan Agreement form.

The Learning Commons staff will verify the information provided in this form with the appropriate faculty member or sponsoring group.

A copy of your syllabus must be emailed to learningcommons@odu.edu before your request is considered. The information in the syllabus must match the information that was provided in the request.
