Welcome to The Illinois DHS/DRS Electronic Visit Verification (EVV) Survey
Effective January 1, 2014 all Homemaker and Home Care Agencies will be required to use the EVV system for their HSP customers. To support the implementation of the EVV system, Sandata needs to confirm and collect some key information about your agency in order to set up your Santrax telephony account. It is required that you complete this survey by September 27, 2013.
This survey will take approximately 5 -10 minutes to complete. An agency needs to complete this survey only one time even if the agency operates in multiple locations or provides services under multiple programs. Before you proceed, please have the following information available to complete this survey:
•Primary and Secondary Contact Information for each location; address, phone, email
•Name of scheduling system(s) used in your agency if applicable
•Technical Person/Administrator in your office
•Number of DHS/DRS Customers
•Number of Staff in your agency
If you have more than 4 locations that will be participating in Illinois DHS EVV, after completion of this survey, please send an email to firstname.lastname@example.org with all of the information requested on the location page for each additonal location (for locations 5, 6, 7 etc.).
