You are invited to participate in a study concerning the effects of the Holocaust upon mothers and their second-generation offspring. My name is Audrey Freshman and I am conducting this research in partial fulfillment of my doctoral degree at The Silver School of Social Work, New York University. I am also a second-generation child of a Holocaust survivor. I am interested in advancing research on the use of alcohol and other substances within the Jewish community in response to trauma.

Would you be willing to take approximately 15 to 20 minutes to fill out the questionnaire attached to this introduction? Your responses will be anonymous; there is no way for me or anyone else to know who completed a survey. Survey data will only be reported in aggregate form, none of the responses of any one individual will ever be published.

The level of risk attached to participating in this study is minimal and is the same as you would experience in your every day life. You can exit the survey at any time and do not need to complete any question that makes you uncomfortable.

If you have any questions please contact me directly by phone: 516 678-2549 or email: afresh314@gmail.com. You can also contact Dr. Lala Straussner, DSW, the Dissertation Chairperson, at Silver School of Social Work at New York University, by phone: 212 998- 5947 or email: sls1@nyu.edu. If you experience any discomfort in response to any question you can go directly to the Resources for Children of Holocaust Survivors for local support groups. http://www.judymeschel.com/coshsuprt.htm#cybersupportgroup

For questions about your rights as a research participant, you may contact the University Committee on Activities Involving Human Subjects, New York University, ask.humansubjects@nyu.edu or (212) 998-4804.

If you choose to participate, please click on “I CONSENT” at the bottom of the screen to begin the survey.

Participation in this study is completely voluntary.

A $5.00 contribution for each completed survey will be made to the United States Holocaust Museum. Although you will not gain anything personally, you will be helping to make an important contribution to the advancement of knowledge about the effects of trauma upon future generations. If you are interested in the research outcome, a summary of the results will be posted on www.afreshstartonline.com.

Thank you in advance for your participation.
