The Northeast Seattle Coalition to Prevent Underage Drinking is interested in your opinions about issues of underage youth alcohol use in our community. This coalition is part of the SPF-SIG grant from the Division of Alcohol and Substance Abuse, which is a 5-year grant awarded to twelve middle schools across Washington.

Your opinion is important to us. It will help us plan effective underage drinking prevention programs in our community.

We would appreciate if you could complete this 10-15 minute survey. We ask that each adult in your household completes the survey. We know that individuals might have different opinions about the issues on this survey, so we would like all adults to complete their own surveys.
