NO/AIDS Task Force, a non-profit HIV/AIDS service organization in the New Orleans area, is conducting a statewide needs assessment for the Men who have Sex with Men (MSM) community. This survey will take approximately 20-30 minutes to complete.
This survey is confidential. This means that we are not asking for your name. No personal identifying information will be collected. Some of the questions are very personal and we want to keep your responses anonymous to protect your privacy. All information provided is voluntary.
The purpose of this project is to find out about the HIV prevention needs for the MSM community in Louisiana. Your participation will contribute in creating a safer and healthier environment within your community.
Please note that some of the questions ask about personal information, including sexual behavior. There is a possibility that answering these questions may cause you some discomfort or embarrassment.
If you qualify to take the survey and you complete the survey, you can enter for a chance to win 1 of 5 iPod Touch devices. Instructions on how to enter will be given at the end of the survey. Only completed surveys will be considered eligible.
You have the right to refuse to participate in any part of the survey with out any penalty. If you do not wish to complete this survey, you can stop at any time and your information will not be used for our project.
Any contact information you may choose to provide is at your discretion. All contact information provided will be stored on a separate password protected data file to ensure confidentiality. Your contact information will be discarded at the end of this project.
By completing this survey, you give permission for your answers to be used for research purposes only and the information will be reported in a manner that respects your confidentiality and privacy.
