Thank you for taking time to answer questions about Canine Leptospirosis in Illinois. The Illinois Veterinary Medical Association has teamed with Fort Dodge Animal Health, through an educational grant to survey veterinarians throughout Illinois about the incidence of lepto in their clinic and local area. We will share the results with you through the ISVMA newsletter.

Some helpful tips:
-- The survey should take only a few minutes.
-- Please report cases in your clinic for the past THREE years
-- Only one response per clinic, please.
-- The survey will close at midnight on August 1.
-- All responses to the survey (one per clinic) will be entered into a random drawing. Responses must be received by Saturday, August 1, to be eligible. The drawing includes five items valued at $100 or more with the top incentive being a 32GB iPod touch.

Again, thank you for your time.
